The summary shows the pledge action choice on the left and the date of service you complied on the right.
As you can see none of the employee's pledges have been loaded into the Wellness Summary yet.
Please note: When you take action on one of your Wellness Pledges it takes at least one month for that information to load into your summary.
Below is the schedule of file uploading into the North Shore-LIJ platform from our carriers:
Health Assessment: 3rd Monday of month
United Healthcare claims: 3rd Monday of month
Cigna Dental claims: 1st Monday of month
Davis Vision claims: 1st day of month
How Your Information Loads
Let's give some examples:
Your annual exam was on February 4, 2015. You should see your visit as complied the following month on the third Monday, March 16. United feeds their files on the 3rd Monday of the month.
Your dental cleaning was sometime in January and again in July. You should see your dental visit as complied on the January date of service. Your second visit will not override your first.
Your eyes were examined on January 26. To view your date of service on our Wellness Summary you will have to check back March 1, first day of the following month.
Still Don't See Date of Service?
Please keep in mind your physician's office may delay filing the claim with the carrier, which will further delay the capturing of your visit. Not to worry - as long as you complete your four wellness pledges/actions before December 31* you will receive the paycheck credits in 2016.

*To receive your credits beginning with the first pay cycle of 2016, finish all pledges before October 31!

There have been a few comments sent to the Total Rewards Blog regarding the timeline of the Pledges. The reason behind the September 30 deadline is so that you can view your compliance on your personalized Wellness Summary before Open Enrollment on November 10. If you have health related appointments scheduled prior to September 30 you will see your date of service in your Summary in October/November and receive the paycheck credits beginning in January 2015.
I had my annual exam in December, 2013, but it doesn't show on my Wellness Pledge Summary. Is there something that I can do to get credit, short of getting an annual exam before September 30?
In response to the above comment August 25 at 10:44 am: Your exams and screenings need to take place in 2014 between January 1 and September 30, therefore your December 2013 annual exam will not appear as a complied pledge on your Wellness Summary. You should be able to complete four pledges by September 30: today you can do the Health Assessment and view the Wellness Video. Another two valuable actions can be a dental and eye exam. Thanks for your healthy interest in this program!
As I understand it, in the table provided above with the different pledges, out of the 10 listed, one must complete at least 4 to continue to receive paycheck credits. Of the 4, one MUST be the online health assessment. Is this correct?
In response to September 7 at 9:18 am: Yes- you must complete and submit the online Health Assessment in order to receive paycheck credits in 2015. Each pledge is worth $260. The maximum amount of paycheck credits you can receive in 2015 is $1,040 (which is in exchange for the Health Assessment and an additional three pledges). I hope this answers your question.
I did my online health assesment on august 22, 2014 and have proof of it, as of today Monday Sept 15 the date still doesnt show completed when I log in to check my Wellness Pledges. What can I do at this point if the online health assesment does not show completed. Ive done it twice to make sure and still nothing shows up .
In response to September 15 at 3:51 pm: Your confidential online Health Assessment is administered by United Healthcare. Participant's submission date, or date of service, is feed to the Wellness Summary platform on the 3rd Monday of the month - which means you should see you compliance on September 22. Monday, September 1 was a legal holiday therefore was not counted as the first Monday of the month. Hope this helps to clarify.
