Let's review the Pledge deadlines:
- Complete the online Health Assessment on myuhc.com and 3 additional pledges by October 31 and you will receive your maximum paycheck credits beginning with the first pay cycle in 2015
- Complete the online Health Assessment on myuhc.com and 3 additional pledges after October 31 (in November/December) and your paycheck credits will still appear in 2015 but they will be delayed 4-8 weeks - keep in mind, they will not be retroactive!
What you can do NOW - if eligible for the Pledge Program*
Step One:
Complete your online Health Assessment on myuhc.com - it's the gateway pledge and is required in order to receive any paycheck credits in 2015
Step Two:
Watch the Wellness Video - it was posted on your iLearn Module for viewing and submission
Step Three:
Schedule 2 appointments (pledges) before Oct 31 and you will receive full credits:
--Annual Exam or Physical
--Dental Exam and/or Cleaning
--Vision Exam
--Breast Cancer Screening/Mammography
--Skin Cancer Screening/Check with a Dermatologist
--Colorectal Cancer Screening
In Addition:
If you currently have your spouse covered under our medical plan you are also eligible to receive credit if he/she completes the online Health Assessment on myuhc.com.

*You must be a non-union benefits-eligible employee or a Lenox Hill Hospital RN who is planning to enroll of re-enroll in North Shore-LIJ's Value or Buy-Up medical plan with United Healthcare to be eligible for the paycheck credits. New hires and those new to the medical plan in 2015 will have 6 months from their benefit's eligibility date to comply with the Program.

Do we only get full credit in 2015 if we complete all the 3 steps on this page (complete the health assessment, watch the video, schedule 2 appointment)?
I don't need a cancer screening and I'm not due for an Annual Exam or Physical. If I only have a Dental Exam and/or Cleaning do I get full credit in 2015?
MEETH has a free Vision Exam - is there a way to 'count' that as the 2nd appointment I would need? Thanks
I response to October 4 at 9:10am - Yes - you will receive full credit for taking action on 4 pledges as stated. In addition to your Health Assessment and the Viewing of the Wellness Video on iLearn, your 2 "clinical pledges", the free Vision Exam at MEETH and the Dental Cleaning Exam, will count. Is my spouse required to do the HA on uhc.com? I already did mine and did all my pledges.
In response to the above comment on Oct 28 at 9:14 am - No, your spouse is not required to complete the Health Assessment just you (the employee) are. The online Health Assessment was required in last year's program but in the new program it is one of the nine choices you have to choose from. Sounds like you are all good to go. Best of health!
I am not due to go to doctor or dentist until November based on my last appointments. Does that mean I will not qualify? I have appointments already set up for November, but based on insurance I couldn't go in October.
I only had one dental exam this calendar year, the next one will have to be done after November. Does this still count?
In response to the above comment Oct 28 at 9:30 - Yes, you qualify for the paycheck credits in 2015 but those two pledges may be delayed by a few weeks. The sooner you go to your wellness appointments the sooner your paycheck credits will start in January. FYI - You are allowed 2 dental exams/cleanings per year so if you have had an earlier one that counts. Also United allows a physical once every calendar year, so if your last visit was November 2013 you can schedule another annual anytime in 2014.

I would like to comment on anonymous post on October 28, 2014 at 9:30 AM. I also fall into this category, and have every in the past. United Health Care does not allow you to get your wellness exam until exactly one year to the day after the previous year. The doctors office does not allow it either. Every year the same thing happens to me in which I cannot schedule my wellness exam until December 3rd.
It would be nice to know that the folks who fall into this category do not have to have a lapse in wellness credits. NSLIJ please consider this an option, its only fair we do not have a lapse in wellness credits. We are still valued employees who are stuck in this 'Post October 31 scheduling conflict".
In response to Post October 31 Scheduling Conflict: United Healthcare covers your Annual Exam on a per calendar basis. If you had an exam anytime in 2013 - you are covered for another one in 2014 - at any time.
The reason the Program is trying to capture Wellness prior to October 31 is to ensure a smooth transition between the provider's office, the carriers that submit the date of service to our HR Information Tech Team and our payroll department. The newly designed Wellness Program promises to reward those who act on their preventive care all year long moving into 2015. Thank you for your interest and patience in this valuable new program.
Is there a way we can confirm that the wellness pledge, video, appointments etc. we made have been counted?

Hi! I'm not sure I understand this make 2 appointment before October 31st. I made an appt with my OB/GYN in January and teeth cleaning in December, does that count? My Health Assessment isnt due until January of next year it says and I have 7 completed pledges.
In response to Suzanne: Your Health Assessment (HA) must be completed in 2014 in order for you to be eligible for any credits. Since you are unable to access your HA that means you did it in 2014. The 2 clinical appointments will credit your 2015 paychecks - but the Dental pledge (worth $10 per paycheck) will be delayed 4-8 weeks because you scheduled in December. Keep in mind you are covered for 2 dental exams and cleanings every calendar year. Refer to your Wellness Summary when logged into Employee Self Service.

Response to Nov 1 at 7:33 am: If you are under your husband's plan the Dental Pledge will not pertain to you. If you have additional questions about forms, contact the HR Service Center at 516-734-7000.
