The Gateway to Your Wellness Pledge Credits

If you are planning to enroll or re-enroll in the North Shore-LIJ Value or Buy-Up medical plan you are eligible to receive 2015 paycheck credits in exchange for completing an online Health Assessment and 3 additional health-related actions.

Health Assessment is the Gateway Pledge
It's not too late to complete the Health Assessment Pledge, which is the most important pledge. Without this pledge you are not eligible to receive paycheck credits for the others.
Example: Susan had a dental cleaning in March, an annual physical in June and a mammogram in August - if Susan completes the Gateway Pledge by October 31 she will receive the maximum amount of paycheck credits ($1,040) beginning with her first pay cycle in 2015. If Susan waits and does not complete her final Pledge until November or December - her paycheck credits will be delayed 4-8 weeks. If Susan ignores the Health Assessment altogether she will not receive paycheck credits for any pledges in 2015.

  1. Visit myuhc.com
  2. Register and log in with a username and password
  3. Click on the Health Assessment tab on the right side of page
  4. Register and login using the same username and password you used for myuhc.com
  5. Answer security questions
  6. Fill in the health information
  7. Hit submit
The Health Assessment is 100% confidential. Your health information is housed by a third party provider, Optum Health and not shared with North Shore-LIJ Health System. The only information the health system receives is the date you submitted it.

Wellness Summary
The Health Assessment will appear on your Wellness Summary, the third Monday of the following month in which the HA was completed and submitted.

The Program's Goal
The Wellness Pledge Program's goal is to educate and engage participating employees about the importance of their preventive healthcare routine. The Program runs on an annual basis and requires health-related actions the year prior to the receipt of paycheck credits. New Hires and those new to the medical plan have six months to comply. How will I know if I completed the necessary qualifications in order to get my credit?

In response to the above comment Oct 28 at 9:22 am - Scroll down the Blog to post on August 14. This post explains the Wellness Summary (which is housed on Employee Self Service when you are logged in). The Wellness Summary captures your date of service approximately 4-8 weeks after your visit. United Healthcare dates are loaded the third Monday of the month, Cigna Dental and iLearn's Video - the first Monday of the month, and Davis Vision the first day of the month. Hope that helps!

If just did the wellness video which was the only thing I was missing, will my payments be delayed because iLeanr doesn't update until Mon, Nov 1st?

In response to Oct 28 at 10:34 am - Since you viewed the video before the deadline of Oct 31 you will see your credit on the first pay cycle in January, although you will not be able to see it reflected on your Wellness Summary until November 3.

How do I ensure my physical and dental exams show up on the benefits summary??

In response to multiple comments regarding - how do I know my pledges were captured: Refer to Blog Post on August 14 which explains how your dates of service/pledge actions are captured.
A quick summary - when you visit your dentist or doctor a claim is then sent to our carriers, United Healthcare, Cigna Dental and Davis Vision. That may take a day or a few weeks. Once the claim is sent from the office the carrier loads the date-of-service (not your health information) to North Shore-LIJ Human Resources IT Team. The carriers feed this information on a monthly schedule: United Healthcare sends the file the third Monday of the month, Cigna the first Monday of the month, and Davis Vision the first day of the month. For example if you did your Health Assessment on Oct 29 you will not see it on your Wellness Summary in ESS until November 17. The Health Assessment submission dates are sent from United Healthcare the third Monday of the month. Hope this helps!

When will information regarding 2015 benefit options be available? My spouse has to pick his elections this week and it is hard to make choices without knowing what my choices are. Even if we cannot select, it would be good if we could see the book. Please advise.

In response to November 6 at 6:44 am - Information regarding OE and 2015 Benefits will be available to all eligible employees Thursday, November 13. If you are currently comparing the North Shore-LIJ plans with a spouse's plan during their employer's open enrollment, rates are available as of Friday, November 7. 11/6/14
I saw the dermatologist on 9/10,
on neither the 3rd Monday of Sept. or Oct. did the pledge show up as completed. (On time, I might add). Now do I end up being penalized? (Probably, right?)

In response to November 6 at 10:23 am - If you visited your dermatologist on 9.10.14 you will receive the paycheck credit beginning with January's 2015 pay cycle. If your pledge does not appear in your Wellness Summary by December, contact the HR Service Center with proof of your visit. Thank you for your patience.

Quick question, I went to one of the Urgent Care Centers and from there they sent me to the ER. I have rec'd. my $100 bill for the ER, should I now expect one from the Urgent Care Center as well? I just want to be prepared $$$.

1-12-15
I went for my wellness visit (yearly physical) in Dec. 2014. At my age, they do an EKG. My doctor takes UniteHealthCare, but is not part of NS-LIJ. I have gone to him for 12 years and do not want to change. I had the understanding that the cost of the wellness visit was covered. Now I receive a bill for the EKG. If my doctor had been part of NS-LIJ, would the EKG also have been covered in full or would I still have rec'd. a bill?
I really feel like I am being nickled and dimed as they say. I hope you dane to answer my question. Thank you.
