Wellness Summary
Our carriers: United Healthcare, Cigna Dental, Davis Vision, and Transamerica transmit your date-of-service (not your health information) into your Wellness Summary, which can be found under the "Personal Information-SelfService-Benefits" tab when you login to Employee Self Service.
Note:
Your Wellness Summary will not be updated for viewing for approximately 6 to 8 weeks post your action.
If you have completed your pledges by the final deadline of Dec 31, 2015:
There is no need to call-in, fax or file claims, office visits or other documents to the HR Service Center, you have complied!
Rest assured:
If you completed your actions before Oct 31, 2015 your paycheck credits will begin with the first pay cycle of 2016.

- Click on the "Health Assessment" on the far right side of screen
- Click on "Take the Health Assessment"
- Complete security questions and health information
- Hit "Submit"

- You just earned yourself $260!

Login to nslij.trsretire.com - once you hit the blue tab "view" a communications box will appear underneath "Your Retirement Outlook"
Select the "Financial Wellness Presentation" and watch in its entirety to receive the credit

What if someone has nslij value plan under their own name and dental and vision under spouse? Can they submit their dental (nslij cigna) and (davis) vision visits to claim the credit? The wording of the wellness pledges mandates the requirement to have a medical plan under your name. It does not specify that for the dental and vision visits, the employee should be primary policy holder.

Yes, your dental and vision visits will be counted towards your wellness pledge credits.
The reason behind the requirement of enrollment in the Value/ Buy-Up United Healthcare plan is to comply with the online Health Assessment (HA) on myuhc.com. The completion of the HA is necessary in order to enter into the program for credits.
The health system’s goal is to improve the health and wellness awareness of its employees, and we feel the Health Assessment can help to accomplishes that.
I work for NSLIJ Flex Staffing. I am trying to take the health assesment but I am not offered United Health and can not log in. Is it still possible for me to enroll and take the health assesment ?

If breast cancer screening and physical were done with out of network providers using spouses medical coverage, can they be used and if so, where do we send information to show that this was done?
You must be enrolled in the North Shore-LIJ Value or Buy-Up medical plan to participate in the Wellness Pledge Program in exchange for paycheck credits in 2016. Your spouse's plan is not a part of the program.

Let's tackle how you view your pledge compliance first:
You can check up to see if your compliance is complete on your Wellness Summary, located on the health system's my Self Service. Login with your universal password and ID - then click on Personal Information>Self Service>Benefits. Click on the "4 more... " and you will find your way to your Wellness Summary.
Keep in mind: If you recently completed a pledge - it will not appear as complied in you summary for at least 6 to 8 weeks of your actual date-of-service.
Your Spouse's responsibility: Your spouse or other dependents are not required to complete any of the pledges.
BUT - in alignment with the program, if your spouse is covered under the UHC plan, he/she can take the online Health Assessment and you would receive $260 in 2016 for that action.

When you say that the above mentioned wellness actions need to be completed by October 31st, did the actions have to be completed within the 2015 calendar year? I ask this because I had my vision checked in December of last year, so I am not due for another appointment yet. Would that count toward one of the actions?

If you happen to complete a pledge by the first deadline of Oct 31, your credit for that pledge will start with the first pay cycle 2016. I see that nothing is stated about the new CareConnect plan. Does this mean that I will not be recieving credits since I switched to CareConnect?
In response to December 30 at 9:22 am: If you enrolled in the CareConnect Employee Plan as your medical plan choice in 2016 you will have the opportunity to save on your benefit costs by completing four Wellness Pledges in 2016. The Annual Pledge Program for 2015 closes out on December 31 and the new program opens January 1, 2016. Complete four pledges in 2016 and you will receive the paycheck credits in 2017.
