New Tax Form(s) Under ACA

A new form (1095-C) will be arriving to your home address early April, from Northwell Health. The purpose of the form is to show the months in which you and your dependents were offered, or covered by, an employer sponsored health insurance plan. This information is reported to the IRS by your employer (Northwell Health). The 1095-C is for your records. This form states that you were covered (or not covered) under the employer sponsored plan.

To further explain, under the Affordable Care Act (ACA) two new mandates were created:
  1. Employer to send 1095-C Form to employees - save for your records 
  2. Employer to send 1094-C Form to IRS
Note: the two forms contain the same information.

Union 
Employees covered by union multi-employer plans will receive 1095-B Form from the union and a 1095-C Form from Northwell Health.

My accountant states that i need this form in order to file my taxes, Is this true? how so if a copy was already sent to the IRS.

In response to C.A. March 4 at 10:55am: You do not need to file the form with your taxes. When you receive the form - be sure to file it in a safe place for reference.

Hi - In response to March 21 at 8:28am: The Northwell Health HR department will be sending the 1095-C Forms out to employees in batches at the end of March through the beginning of April. You can go ahead and file your taxes for 2015 - you DO NOT NEED THE 1095-C FORM to file your taxes this year. When you receive the form, simply file it away.

Will a corrected form be issued (re: middle initial error) for filing with the 2016 taxes? Could this form be issued on or about the same time as the W2's? That would be most helpful for early filers.

In response to March 30 at 9:37am: You will only receive the one 1095-C Tax Form in 2016. The error with the middle initial will not affect you in any way - as you do not have the file this form in 2016. Next year the form will arrive earlier for tax filing purposes. I hope this additional explanation helps.

I too am getting told by our very experienced accountant that we need proof of coverage from Northwell, the 1095 C or 1095 B for last year July 2015- Dec. 2015

In response to April 1 at 1:09pm: You do not have to wait for your 1095-C Form to file your taxes in 2016. Because the form is new this year, the government is granting a one year grace period. If you uncomfortable and would prefer to wait for the form you should receive it in the next week or so.
When you receive the form - simply file it with your records. Next year the form will arrive earlier for those who like to file well in advance of the April 15 deadline.

My comment code is 1h which means "no offer of coverage". Could this be because I receive my health insurance through my Union NYSNA and not NSLIJ? I'm just trying to understand the wording ....

Does anything need to be done if your 1095-C form has both incorrect middle initial and incorrect company address?

In response to April 5 at 9:23am: Northwell Health is aware of the error, listing all eligible employees with a middle initial "P". Since there is no action necessary with this form in 2016 the error will not be corrected.
The NYC return address was due to the fact that a third party released the mailing. To confirm: there is no action necessary on your part this year. Thank you for your interest.

In response to Dbano on April 5 at 9:01am: You are correct! Since you receive your health insurance through NYSNA your 1095-C form will state "no offer of coverage" from Northwell Health. Be on the lookout for a 1095-B form from your union - this will state that you were offered coverage through NYSNA in 2015. There is no necessary action needed in 2016.

In response to "unknown" April 8 at 7:59am: Considering there a grace period of one year regarding the new 1095-C Form it does not matter that your name is incorrect on this year's form. If your name is correct on all other Northwell Health documents there is no need for concern. To be sure all your information is correct, login to Self Service and check your personal information.
